Abstract

New York City (NYC) has emerged as one of the epicenters of the current SARS-CoV-2 pandemic. To identify the early transmission events underlying the rapid spread of the virus in the NYC metropolitan area, we sequenced the virus causing COVID-19 in patients seeking care at the Mount Sinai Health System. Phylogenetic analysis of 84 distinct SARS-CoV2 genomes indicates multiple, independent but isolated introductions mainly from Europe and other parts of the United States. Moreover, we find evidence for community transmission of SARS-CoV-2 as suggested by clusters of related viruses found in patients living in different neighborhoods of the city.

Few things are more frightening as a weapon than infectious diseases. The idea of a creeping, invisible death that will probably make you bleed from places you'd rather not is much worse than the idea of getting shot.
In this veteran's opinion, anyway.
In any case, it's a good thing the US Army has its Medical Research Institute of Infectious Diseases to counteract the development or weaponization of superbugs and viruses for use against the United States.
Since the 1960s, USAMRIID has been on the front lines of disease research and testing in order to keep the communists, terrorists, Mother Nature or anyone else from using biological weapons against America and its allies.
It's one of the only places in America that can handle Biosafety Level 4 viruses, the highest levels of safety containment for some of the world's deadliest diseases and toxins.
Here are just a few of the menaces USAMRIID is helping to nip in the bud before they get out of hand or into the wrong hands.
Outbreak Of Hanta Is Unusual

Michael_Novakhov shared this story .

A hantavirus outbreak killed 11 people and sickened nine others in Argentina last year in the first known case in which the respiratory illness spread from person to person, United States health officials have reported.
Scientists had previously thought that hantavirus pulmonary syndrome could sicken people only if they inhaled microscopic bits of rodent urine or feces containing the virus.
No cases of person-to-person transmission have been documented in the United States, where the viral strain that causes respiratory illness was first identified in 1993. Virtually all cases in this country were linked to exposure to the feces or urine of infected deer mice.
''We are hoping physicians will be looking for this,'' said Dr. C. J. Peters, chief of the special pathogens branch at the Centers for Disease Control and Prevention in Atlanta. ''We also will be looking for this in our central registry of cases.''
Hantaviruses are a family of viruses named for the Hantaan River in Korea, where the first strain was discovered decades ago. That strain infected about 3,000 American soldiers during the Korean War, killing 190 by destroying their kidneys.
There are dozens of strains throughout the world, but none until now have shown that they could spread from person to person, Dr. Peters said.
Hantavirus pulmonary syndrome first emerged when it killed dozens of people in the Four Corners region, where Arizona, Colorado, New Mexico and Utah meet. Since then, hantavirus has sickened 162 people in 27 states, and 76 of them have died, the centers said.
The respiratory illness starts out with flulike symptoms that worsen and can become deadly. The lungs drown in fluid and the heart slows down, sending patients into shock. About half of people infected with hantavirus die.
In the coastal town of El Bolson in southern Argentina, 18 people became sick from September to December with hardly any rodents around. In addition, two more people who had contact with El Bolson residents but never went there became ill.
A 41-year-old man was the first to get sick. In three weeks, his mother and his doctor fell ill. Four weeks after that, the doctor's wife became sick, and it spread from there. The doctor's wife traveled to Buenos Aires for medical care, and then her doctor got sick. When the daughter of the first man's maid got sick after riding in a car with her, scientists knew this was no ordinary hantavirus outbreak.
''We first thought: could the rodents get into the car?'' Dr. Peters recalled. ''The Argentines took that car apart looking for rodent nests and couldn't find anything.''
Health officials put out traps for rodents in the area where the first patient lived and found only a few. The man had moved into a new, brick home from a wooden cabin several weeks before he got sick.
''We believe the virus ultimately came from a rodent, but it appears it may have been passed on through several persons,'' Dr. Peters said. Three of the ill were doctors who treated the hantavirus patients.
The agency in Atlanta has kept close tabs on health care workers in the United States since the disease first emerged. In 1993, blood tests of 396 workers, 266 of whom were exposed to hantavirus patients, found that none were sick. ''There is not a single case in the United States or Canada yet where there is a smoking gun,'' said Dr. Brian Hjelle, a molecular epidemiologist at the University of New Mexico.
The only case that came close in this country was in Crownpoint, N.M., in 1993 when four adults living in the same trailer got sick. But the trailer was overrun with rodents, said Dr. Fred Koster, who cared for the first hantavirus patients in whom the disease was diagnosed in New Mexico. ''We have to ask ourselves, why is Argentina's virus different from ours?'' Dr. Koster said. ''That simply remains a puzzle.''
Wild rats, laboratory rats, pet rats: Global seoul hantavirus disease revisited UTMB Health Research Expert Profiles

Michael_Novakhov shared this story from Publications - UTMB Health Research Expert Profiles.

Abstract

Recent reports from Europe and the USA described Seoul orthohantavirus infection in pet rats and their breeders/owners, suggesting the potential emergence of a new public health problem. Wild and laboratory rat-induced Seoul infections have, however, been described since the early eighties, due to the omnipresence of the rodent reservoir, the brown rat Rattus norvegicus. Recent studies showed no fundamental differences between the pathogenicity and phylogeny of pet rat-induced Seoul orthohantaviruses and their formerly described wild or laboratory rat counterparts. The paucity of diagnosed Seoul virus-induced disease in the West is in striking contrast to the thousands of cases recorded since the 1980s in the Far East, particularly in China. This review of four continents (Asia, Europe, America, and Africa) puts this emerging infection into a historical perspective, concluding there is an urgent need for greater medical awareness of Seoul virus-induced human pathology in many parts of the world Given the mostly milder and atypical clinical presentation, sometimes even with preserved normal kidney function, the importance of simple but repeated urine examination is stressed, since initial but transient proteinuria and microhematuria are rarely lacking.

Koreans rush to get rodent bile

A worker for the Nakdong River Basin Environmental Office hoists a nutria in Gimhae, South Gyeongsang. [JOONGANG PHOTO]
Tuesday was no ordinary day for the Nakdong River Basin Environmental Office. Its telephone line was jammed with over 100 callers all eagerly asking the same question: how do I trap a nutria?

It was to the point where we couldnt get any other work done, explained Jeon Hong-won, an official on the offices Nutria Extermination Team. Throughout the day, people were asking me how to trap the animal and whether their bile was up for sale.

They had every reason.

In a joint study by Gyeongsang National University in Jinju, South Gyeongsang, and the Nakdong River Basin Environmental Office, the rodents bile was recently found to have a higher percentage of ursodeoxycholic acid (UDCA) than that of a bears, prompting what now seems to be a rush to get ahold of the species.

UDCA is known to be effective for treating liver ailments, especially primary biliary cholangitis (PBC).

According to the research, a nutrias bile is 43.8 percent UDCA, higher than that of an American black bear (38.8 percent), a brown bear (18.6 percent) or a badger (4.5 percent).

What many seem to have overlooked, however, was that the substance is nowhere near being edible.

Prof. Yeon Seong-chan of Gyeongsang National University, who participated in the research, warned that consuming a nutrias bile can cause serious parasitic infections.

The Korean government encourages nutria hunting with a 20,000 won (about $17) bounty for each catch, but said it would strictly prohibit nutria farming due to such health concerns. Anyone who violates the law prohibiting the trade, storage or breeding of nutria is subject to a maximum fine of 20 million won.

The national nutria control program is aimed at alleviating the damage they cause to wetlands.

According to descriptions from the Washington Department of Fish & Wildlife, nutria rodents are semi-aquatic, native to southern parts of South America and three times the size of muskrats. Adult nutria are about 24 inches long from nose to tail, with the tail itself being 12 to 16 inches long.

Rodents such as nutria, rabbits, hares, voles, muskrats and beavers are some of the species that can be infected with the bacterial disease tularemia. Tularemia may be transmitted to humans if they drink contaminated water, eat undercooked, infected meat, or allow an open cut to come into contact with an infected animal.

The Hypothesis of the Disease X-19 as the New Abwehr Intelligence Operation and the new type of the combined Biological - Informational Warfare

Disease X-19, presently designated as "Covid-19" is the complex and long standing, about 2-3 years duration, combination of the biological and the informational warfare, conceived, operationalized, and managed by the New Abwehr, to commemorate the 75-th anniversary of the end of the WW2.

It started in 1917, and is the direct sequel of the Operation Trump: the managed and manipulated election of Donald Trump as the US President. The Russian Jewish Mafia are the foot soldiers and the immediate executors for the New Abwehr in both operations.

Disease X-19 is the combination biological weapon. The main and the most deadly part of it is the Hantavirus Infection, transmitted by rodents, with the "novel Coronavirus", Sars-Cov-2 as the second part and the cover, designed to facilitate the infectivity of the main Hantavirus part, and to make the attempts to arrive at the correct diagnosis confusing and more difficult.

The emergence of the Disease X-19 Pandemic was promoted by the unprecedented and orchestrated mass media campaign, controlled by the New Abwehr. The initial "legend", the explaining story, was constructed around the emergence of the epidemic in Wuhan, China, and reflects the today's geopolitical underpinnings and the ease of the manipulation of the Chinese medical thinking and the public opinion.

Apparently, the Chinese epidemic was first seeded in 2017-2018 by infecting the huge and commercially competitive livestock of the domestic pigs with the Coronavirus, Hantavirus, and the African/Classical Swine Fevers, with the latter served as the manifest pathological cover for the first two infections.

About half of the Chinese pigs livestock was culled, and most likely the carcasses were not disposed of properly, providing the abundant feeds for the rats and mice, and infecting them with the pathogens listed above. It is also quite possible, and even likely, that the significant part of the infected pork and pork products were sold at the discounted prices and shipped broadly worldwide, providing the operators and the executors of the Disease X-19 future pandemic with the ready made, natural  and accessible infectious material in unlimited supply.

My assumption and the part of this hypothesis is, that this infectious pork was then broadly distributed and dispersed in the major capital cities around the world, by adding them discreetly, in the sealed bags, to the street garbage piles, thus infecting the rodents with these and possibly other infectious diseases.

The recent, last 2-3 years especially, explosions of the rodent populations, including the unusually large and possibly "transgenic" rats in the major world capitals and the other large cities is the well known fact. It is logical to assume that these explosions of rat and mice populations are due to the generous feedings of the infected pork as the part of this operation and the warfare.

It appears that the clinical presentations and manifestations of the Disease X-19 are quite diverse, polymorphous, multi-system and multi-organs, and they vary in severity from asymptomatic to lethal pulmonary, kidney, and other conditions. This complex clinical picture can represent several different illnesses.

The testing and evaluations for the infections other than presumed "novel Coronavirus", the Sars-Cov-2, are not performed in the hospital settings, and the clinical diagnostic tests for Hantavirus do not exist. That is why the main culprit can be easily missed. Furthermore, the clinical pictures of the Hantavirus infections and "Covid-19" appear to be very similar, practically the same.

The summary of the Disease X-19 Hypothesis is presented in this post as the brief, general and the somewhat schematic outline. The further details, the causes and mechanisms, both the medical-epidemiological and the forensic-investigative have to be researched and elucidated further.

Renal Manifestations Of The Novel Coronavirus
Renal Manifestations Of The Novel Coronavirus

Michael_Novakhov shared this story from Lipi Roy.

Within the last 48 hours, the United States reached the solemn milestone of surpassing 100,000 lives lost from Covid-19. Sadly, community spread of this novel coronavirus continues as cases and deaths in the U.S. relentlessly rise, now reaching 1.79 million and 104,000, respectively. Multiple clinical studies have also indicated that were dealing with more than just a respiratory virus: neurologicalcardiac and clotting manifestations have been documented thus far. Here, I uncover the effects of COVID-19 on the renal system, gleaning insights from three of the nations kidney experts.
Outside the respiratory system, the kidneys are among the most common organs targeted by SARS-CoV-2. To illustrate the magnitude of the problem, among hospital ICUs in New York City the epicenter of Covid-19 approximately 20-40% of intubated patients had acute kidney injury (AKI) requiring dialysis (described later). Another study conducted by Northwell Health, New York states largest healthcare provider, found that among 5449 patients admitted with Covid-19, 36.6% developed AKI.
What Do the Kidneys Do?
We are born with two kidneys, each made up of a million nephrons which work together to perform multiple life-sustaining tasks. One major function is to act as the bodys filtration system: kidneys remove waste products and excess fluid through the urine. In addition, according to the National Kidney Foundation, kidneys play a critical role in regulating the body's salt, potassium and acid content. Kidneys also produce hormones such as erythropoietin which promotes red blood cell production as well as other hormones that regulate blood pressure and calcium metabolism.
How Does Covid-19 Affect the Kidneys?
Covid-19 has been shown to impair renal function in several ways, ranging from poor blood flow to the formation of tiny blood clots which can clog the kidneys and prevent urine formation.
PROMOTED
By far the most common injury is Acute Tubular Necrosis or ATN, which happens when the kidneys do not get enough blood flow because a Covid-19-infected patient goes into shock and has very low blood pressure, explained Herbert Lin, MD, PhD, FASN, associate professor of medicine, Massachusetts General Hospital and Harvard Medical School. Direct infection of the kidneys is also a possibility.
There is likely a small subset of patients that have direct viral infections of the kidney but this is likely a rare event, said Matthew Sparks, MD, Assistant Professor of Medicine, Division of Nephrology, Duke Medical Center, citing an NEJM study with superb methodology. In a recent paper, Dr. Sparks and colleagues emphasized that AKI can be a severe complication of Covid-19.
Unfortunately, many patients infected with Covid-19 sustained renal injuries so severe that their kidneys almost shut down completely. Dialysis, or kidney replacement therapy, is a procedure where blood is filtered by a machine to clean out the urine that builds up in the blood when the kidneys do not work well, described Dr. Lin.
At the height of the pandemic, New York City hospitals were seeing no shortage of patients requiring dialysis, according to David S. Goldfarb, MD, Clinical Director, Division of Nephrology at the NYU Grossman School of Medicine.
A surprisingly high proportion of the people who needed ventilators to help them breath also had kidney failure that required dialysis, recalled Dr. Goldfarb. In fact, when everyone was talking about running out of ventilators, the medical centers in New York were worried about running out of dialysis machines for that many people. The NYU nephrologist went on to point out that the combination of lung and kidney failure was associated with a very high mortality rate, though a few people did recover. The good news is that when most of these very sick COVID-19 patients recover, so will their kidneys.
What Should We All Know About Kidney Health?
The optimal way to care for our kidneys is by controlling our blood pressure (healthy diet, low salt, regular exercise, quitting smoking) and staying hydrated with water, not sugary beverages or alcohol. There are other methods, too.
Avoid taking medications such as NSAIDs (e.g. ibuprofen) that can harm the kidneys if taken in excess, reminded Dr. Lin. And if you have diabetes mellitus, control your blood sugar as much as possible as hyperglycemia can also cause long-term renal damage.
Dr. Sparks message to physicians: Be cognizant of kidney function and other parameters such as proteinuria and hematuria in patients with COVID-19. He also acknowledged confusion about management of common blood pressure medications such as ACE inhibitors and ARBs (angiotensin II receptor blockers): Currently, there is no evidence that these medications alter the clinical course or lead to an increased risk of infection.
The most sensible way to protect our kidneys, according to Dr. Goldfarb dont get the virus!
Social distancing will prove effective, and wearing a mask cannot be underestimated. Wise words rooted in long-standing public health from Dr. Goldfarb.
******
Other renal health considerations: patients with pre-existing chronic kidney disease will be at increased risk for AKI. In addition, depending on location and status of local hospital staffing, individuals receiving peritoneal dialysis at home may encounter disruption in delivery of PD supplies and nursing support. Kidney transplant programs will also likely be on hold given the multiple resources required including staffing.
Whats become obvious by now is that this coronavirus has triggered multiorgan damage and has catalyzed the global medical research community to examine its plethora of effects. I remain humbled by this pathogen and by sickness in general. To quote the great Canadian physician and father of modern medicine, Sir William Osler: Medicine is a science of uncertainty and an art of probability.

Rats possible culprit behind Wuhan virus outb...

Michael_Novakhov shared this story .

TAIPEI (Taiwan News) As scientists strive to identify the source of the highly contagious coronavirus (COVID-19) that has infected over 75,000 worldwide, a Hong Kong epidemiologist has pointed to a new suspect rats in the Chinese city of Wuhan.
Ng Kam-cheung (), who was involved in the investigation into the cause of SARS in 2003, blamed the rodents as being the vector of the epidemic that sickened over 300 at the Amoy Gardens residential complex, the most seriously affected location in Hong Kong. His study was published in the prestigious medical journal The Lancet.
While Wuhans Huanan Seafood Market, where the illegal wildlife trade was rampant, has been singled out as a possible source of the disease, Ng suspects it may not be the only source. Three of the first four confirmed cases had never visited the market nor had contact with those working there, he said, and an article published on The Lancet on Jan. 24 found that 34 percent of the first 41 cases had likewise not set foot in the market before.
The COVID-19 virus has been detected on surfaces and floors at the market, but no animal has yet been identified as the carrier. One hypothesis suggests that bats, ultimately believed to have been the source of the SARS outbreak, could also have caused the Wuhan outbreak, but Ng questions this, observing that the animal is not active during the winter and that it had not been available at the market when the disease broke out, wrote UDN.
He conjectured that rodents could have been hosts after contracting the pathogen from bats at nearby facilities. This could explain why the virus has spread so fast since rats scurry through commercial and residential areas, according to Ng.
The epidemiologist urged the Chinese authorities to immediately test rodents dwelling in Wuhan for the virus and take the appropriate pest control measures if they are proven to be the culprit behind the outbreak.
The coronavirus could give bioterrorists ideas, security group warns

Michael_Novakhov shared this story .

A security group warns that the COVID-19 pandemic could inspire terrorists to turn to bioweapons.
Why it matters: New technologies like gene-editing have the potential to make future bioweapons deadlier and easier to create, while COVID-19 demonstrates just how vulnerable the world is to any kind of biological threat.
What's happening: On May 25 the Committee on Counter-Terrorism at the Council of Europe issued a statement arguing that the pandemic had raised the risk from bioterrorism.
  • "The intentional use of a pathogen or other biological agent for the purpose of terrorism may prove highly effective and cause damage both human and economic on a far grander scale than 'traditional' terrorist attacks," the committee wrote.
Be smart: Scientists overwhelmingly agree that there is little evidence to suggest that the novel coronavirus was engineered in a lab, rather than emerging naturally from an animal source. But with over 350,000 deaths and trillions in likely economic damage, COVID-19 showed what even a relatively mild but contagious new virus could do.
  • A deliberately engineered and released pathogen would likely be far worse, as I noted in Axios earlier this month. That's because new tools permit the creation of viruses that could be deadlier and more contagious than anything emerging from nature and because a bioweapon could be released repeatedly, foiling efforts at containment.
  • A 2018 pandemic simulation featuring an engineered bioweapon resulted in a fictional global outbreak that killed 150 million people.
What to watch: In its statement, the Council of Europe committee urged a coordinated international response to bioterrorism, including "a common surveillance system capable of detecting suspicious cases."
Reality check: Even though the novel coronavirus is almost certainly natural in its origins blame the bats, probably the global response has been anything but coordinated. It's even more difficult to imagine the world coming together in response to a biological threat that was released deliberately.
Go deeper: The coronavirus pandemic reawakens bioweapon fears
The Council of Europe continues working to enhance international co-operation against terrorism, including bioterrorism

Michael_Novakhov shared this story .

The Covid-19 pandemic has demonstrated the vulnerability of modern societies to viral infections and their potential for disruption. The intentional use of a pathogen or other biological agent for the purpose of terrorism may prove highly effective and cause damage both human and economic on a far grander scale than traditional terrorist attacks, paralyzing societies for prolonged periods, spreading fear and sowing distrust far beyond those communities immediately affected.
The response to bioterrorism must always be a coordinated one. It must mobilise a wide range of human and material resources and therefore require interinstitutional action of the member States of the Council of Europe. The multiplicity and variety of the responders require an interconnection of communication systems, which are essential tools for crisis management. In contrast to the development of exercises in the nuclear field, there is still little feedback in the biological field. The training of civil security and health actors on different types of scenarios is a fundamental element in the effective implementation of a plan to combat biological attacks. The response should also include health and legal monitoring based on a common surveillance system capable of detecting suspicious cases, the reporting of any abnormal, serious or unusual clinical situations and/or the development of epidemiological investigation guides.
All countries are vulnerable to bioterrorism. Its damage is rapid and potentially global. it is necessary to strengthen preventive bioterrorism measures by means of competent interinstitutional intervention and effective international cooperation. The Council of Europe Committee on Counter-Terrorism (CDCT) currently has no concrete evidence of a heightened risk of bioterrorist attack due to the pandemic. However, the experts will continue to work with all our member States to develop common legal standards, facilitate contacts among competent authorities, and organise a coordinated, strengthened response to emerging threats, including those posed by bioterrorism.
* Specific examples of how to anticipate and react to terrorism threats are provided in the Council of Europe Counter-Terrorism Strategy (2018-2022)

Abstract

Many pathogens that can cause major public health, economic, and social damage are relatively easily accessible and could be used as biological weapons. Wildlife is a natural reservoir for many potential bioterrorism agents, and, as history has shown, eliminating a pathogen that has dispersed among wild fauna can be extremely challenging. Since a number of wild rodent species live close to humans, rodents constitute a vector for pathogens to circulate among wildlife, domestic animals, and humans. This article reviews the possible consequences of a deliberate spread of rodentborne pathogens. It is relatively easy to infect wild rodents with certain pathogens or to release infected rodents, and the action would be difficult to trace. Rodents can also function as reservoirs for diseases that have been spread during a bioterrorism attack and cause recurring disease outbreaks. As rats and mice are common in both urban and rural settlements, deliberately released rodentborne infections have the capacity to spread very rapidly. The majority of pathogens that are listed as potential agents of bioterrorism by the Centers for Disease Control and Prevention and the National Institute of Allergy and Infectious Diseases exploit rodents as vectors or reservoirs. In addition to zoonotic diseases, deliberately released rodentborne epizootics can have serious economic consequences for society, for example, in the area of international trade restrictions. The ability to rapidly detect introduced diseases and effectively communicate with the public in crisis situations enables a quick response and is essential for successful and cost-effective disease control.

Here's The Virus That People Are Catching From Their Pet Rats

Michael_Novakhov shared this story from Judy Stone.

In a first, the CDC recently reported an outbreak of a type of hantavirus called Seoul virus among pet rat owners. The first case was in a patient who owned and operated an in-home rattery, or rat-breeding facility, with a family member becoming similarly ill soon thereafter. The investigation found rat Seoul virus infections in 31 facilities in 11 states with human and/or rat infections, and six had exchanges with ratteries in Canada.
Testing found 24 people (13.1%) with Seoul virus antibodies; three (12.5%) were hospitalized and no deaths occurred. The outbreak was stopped by quarantining affected facilities and killing infected rats, as well as boosting cleaning.
Seoul virus infections can range from asymptomatic to fatal, with death from hemorrhagic fever with renal syndrome (HFRS), or bleeding with kidney failure, occurring uncommonly, in 1-2%Infections generally occur from brown or Norway rats (Rattus norvegicus), who may remain asymptomatic though infected with the virus.
Related hantaviruses are far more deadly and common, generally transmitted by deer or white-footed mice.
Hantavirus infection reports are popping up throughout the Western hemisphere. The first case this year, in a middle aged man, was in La AraucanĂa in central Chile.
Four mice have tested positive for the virus this year in San Diego County, though no people are ill yet; they had twelve isolates last year. It can take up to six weeks for symptoms to develop.
PROMOTED
Outbreaks
The first recognized outbreak of hantavirus was in 1993, when there was a sudden cluster of deaths from an overwhelming lung infection in previously healthy young people in the Four Corners area of the southwest.
Investigators looked for the source of infection, and included rodents trapped in and around homes of those infected. They found hantavirus infection in the deer mouse (Peromyscus maniculatus), and the CDC virology branch who isolated the virus named it Sin Nombre virus (SNV); it is the most common strain in the U.S.
What especially intrigued me at the time of this first outbreak was that the clues to the puzzle came from Navaho elders, who had noted similar cyclical clusters of deaths previously. They noted that each cycle followed a year of increased rainfall (El Niño year), which resulted in additional food for a suddenly growing deer mouse population. People living adjacent to the mice then had more exposure, resulting in the spike in cases. This was one of the memorable early studies on climate change and infections.
From 1993 through 2016, there have been 728 infections reported in the U.S. The mean age was 38 years (range, 5-84 years), and more occurred in males. The disease is serious, with fatalities in 36% of those infected.
The other notable cluster was in Yosemite National Park in 2012, among people staying in tent cabins in one section of the park. Epidemiologic investigation found the source to be deer mice nesting in the walls of the tent cabins. Three of the 10 people infected died, and that part of the park was temporarily closed. Complicating the research and tracking was that two of the patients were from the East Coast.
(That emphasizes the value of Infectious Disease training and taking a careful travel and exposure history. Working in Pennsylvania, I picked up one such case in a young construction worker whose job was laying pipes in the ground near Pittsburgh. I was surprised, as hantavirus is uncommon on the East Coast, but was struggling to figure out what was making him ill.)
In 1993, the first case of hantavirus was reported on the East Coast, in a 61 year old man who had been hiking along the Appalachian Trail. He was quite ill with sepsis and multi-organ failure, but recovered. He had noted mouse droppings in shelters he had stayed at.
Several years ago, we started up an old car that had sat for months and, when turning on the heater fan, got blasted with a noxious odor and bits of debris. It turned out that a mouse had made a nest in the engine, and later died there. Fortunately, we didnt become ill, though hanta certainly crossed my mind, and I told my husband to remember to tell the doc to look for hantavirus, should I become ill, since no one here in Western Maryland would otherwise think to look. Last year, other cases of hantavirus from mice nesting in cars were reported from Washington State.
How is hantavirus transmitted?
Hantavirus has a variety of mice and rats as its host. Although the animals may be asymptomatic, they still shed the virus in their secretionsurine, droppings, and saliva. People usually become infected by breathing in air contaminated with the virus. They can also become infected by contaminating their hands with these infected secretions, and then touching their nose, mouth, or eyes (like many other infections). It is not transmitted from one person to another.
Whos at risk?
Anyone coming in contact with urine, droppings, or saliva of infected rodents is at risk, but the biggest risk is from aerosolization of the virus, as occurs with vacuuming or sweeping. Hantaviruses are more common in the southwest U.S., but have occurred elsewhere.
Symptoms
With Seoul virus, the initial symptoms are relatively nondescript, including fever, headache, back pain and nausea developing 1-2 weeks after exposure. Some people develop a rash or eye inflammation. A Tennessee teen also developed bloody urine. More severe cases are associated with shock and hemorrhagic (bleeding) fever and kidney failure.
In contrast, the more common Sin Nombre hantavirus typically causes pulmonary (lung) symptoms and pneumonia. It is far more deadly, killing 38%.
How are hantavirus infections diagnosed?
First, someone needs to think of the disease, which is not likely to happen without a detailed history of exposures. Then diagnosis is made by special blood tests looking for antibodies to infection, available at the CDC.
Faced with a new disease outbreak the The National Center for Emerging and Zoonotic Infectious Diseases (NCEZID) at the CDC actually developed a new test for Seoul virus, enabling rapid testing of breeders animals and helping to contain the outbreak.
The CDC and private labs (IDEXX and Charles River Laboratories) are also now able to test blood from rats for Seoul virus using a polymerase chain reaction (PCR) test, which looks for bits of genetic material from the virus. Rats are asymptomatic, so the only way to know if one is infected is to have it tested. The CDC has contacted those ratteries known to have been connected with a case.
Treatment
There is no standard, effective treatment for hantavirus. Ribavirin may be effective if given early, but that is unlikely to happen unless someone is aware of an obvious exposure or during an outbreak. Treatment is largely supportive with fluids and oxygen.
Cleaning up after rodents
Mice and rats transmit the hantavirus to people through infectious secretions or droppings or aerosolization from bedding that has been contaminated.
The most critical way to avoid infection is to not aerosolize material you are cleaning. This means, dont sweep or vacuum or shake out contaminated materials. Instead, spray the materials 1thoroughly with a solution of 1 part bleach to 9 part water. After five minutes, use paper towels to wipe up the debris. Clean the area again with the disinfectant.
You can follow this for mice in your engine compartment as well, but disconnect the battery cables first. If the mice have gotten in the air ducts, you will likely need a new filter and hoses, and may need a mechanic to help.
Be sure to wash your hands thoroughly when done.
Conclusion
While hantavirus infections are fortunately rare, it's worth while being cautious if you come into contact with mouse or rat droppings. Be careful not to aerosolize any rodent contaminated debris we do come into contact with. And please tell your physician if you have pets or recent travel.

Hemophagocytic lymphohistiocytosis (HLH) is a rare immune disorder in which overactivity of white blood cells leads to hemophagocytosis and can result in death. HLH may be primary due to genetic causes or secondary due to cancers, autoimmune disorders, or infections. Although a variety of infections have been shown to cause HLH, studies have raised the possibility of HLH linked to HFRS, mostly due to PUUV- induced HFRS (Puumala virus) [56].
Although it has been shown that wild Norway rats on the east coast of the United States can carry SEOV, it has never been noted in Washington, DC [78]. This case represents a reported diagnosis of SEOV in a person residing in Washington, DC, and a case of HLH secondary to SEOV. - Domestically Acquired Seoul Virus Causing Hemophagocytic LymphohistiocytosisWashington, DC, 2018 | Open Forum Infectious Diseases

South Korea warns against eating river rats

Michael_Novakhov shared this story from BBC News - Home.

By News from Elsewhere......as found by BBC Monitoring
South Korea's government is warning people not to eat river rats, after a reported surge in the number of people hunting the creatures for their bile.
A university study published in January said that the rodents' gall bladders had a higher level of ursodeoxycholic acid than that found in bear bile, which is used in traditional medicine in parts of Asia. A professor who worked on the study warned that consuming river rat bile was dangerous, but there was a flurry of interest from people wanting to get their hands on one of the animals.
A concerned environment ministry spelled out the risks on Tuesday, telling Koreans that river rats are not safe for human consumption, The Korea Times reports. "The gall bladder and liver can be infected with zoonotic bacteria and parasites," ministry representative Noh Hee-kyong tells the paper.
While the government doesn't want people to eat the animals, it does encourage hunting. River rats are considered an invasive species in South Korea, having only been introduced in the 1980s for their fur, The Korea Times notes. As part of its eradication drive, the government offers a cash reward of 20,000 won ($17; £14) for each one caught.

  1. My life is beautiful thanks to you, Mein Helfer. Lord Jesus in my life as a candle light in the darkness. You showed me the meaning of faith with your words. I know that even when I cried all day thinking about how to recover, you were not sleeping, you were dear to me. I contacted the herbal center Dr Itua, who lived in West Africa. A friend of mine here in Hamburg is also from Africa. She told me about African herbs but I was nervous. I am very afraid when it comes to Africa because I heard many terrible things about them because of my Christianity. god for direction, take a bold step and get in touch with him in the email and then move to WhatsApp, he asked me if I can come for treatment or I want a delivery, I told him I wanted to know him I buy ticket in 2 ways to Africa To meet Dr. Itua, I went there and I was speechless from the people I saw there. Patent, sick people. Itua is a god sent to the world, I told my pastor about what I am doing, Pastor Bill Scheer. We have a real battle beautifully with Spirit and Flesh. Adoration that same night. He prayed for me and asked me to lead. I spent 2 weeks and 2 days in Africa at Dr Itua Herbal Home. After the treatment, he asked me to meet his nurse for the HIV test when I did it. It was negative, I asked my friend to take me to another nearby hospital when I arrived, it was negative. I was overwhite with the result, but happy inside of me. We went with Dr. Itua, I thank him but I explain that I do not have enough to show him my appreciation, that he understands my situation, but I promise that he will testify about his good work. Thank God for my dear friend, Emma, I know I could be reading this now, I want to thank you. And many thanks to Dr. Itua Herbal Center. He gave me his calendar that I put on my wall in my house.
